Patient Journal Documents

Since the 1970’s journals have been used around the world to help patients come to terms with their critical illness. If you have a patient who you feel may benefit from having a journal at the end of their ICU stay, the documents below have been developed to guide you on how to start, maintain and hand over a patient journal.
